All referrals and service request should be made using the forms below. After you have completed the online referral/service request, someone will be in contact with you to proceed with the intake process. You will then receive an intake application packet through our client portal and your initial appointment will be scheduled.
All initial appointments are face to face and held at our office location. Following appointments may be conducted using tele-health methods, if that service is authorized by your insurance carrier and appropriate for your treatment.
Face mask are available for use and you may be asked to wear one if you are at exhibiting coughing or sneezing. If you have been diagnosed or have symptoms associated with COVID-19, or any other communicable disease or contagious condition, please do not visit the office. Contact us and we will reschedule your appointment or conduct it virtually if possible. We value your safety and thank you in advance.
